Thank you for choosing IHS as your primary natural health care provider.  We are committed to providing you with quality natural heath care!  In an effort to familiarize you with our office, below are the office and financial policies.  Please read and sign the acknowledgement.  Thank you.

 

  1. Phones: Telephones will be answered during business hours, which are generally: Monday 9am-1pm and 2:30pm-5pm, Tuesday 10am-1pm and 2:30pm-5pm, Wednesday 9am-1pm and 2:30pm-5pm, Thursday 9am-1pm and 2:30pm-5pm. If there are changes to the business hours they will be posted on the front door and on our website.

 

  1. Scheduling Appointments: We do not accept walk-in patients. You must have a schedule appointment to be seen in the office.

 

  1. Re-Examinations: A re-exam is required if a certain amount of time has passed since your last appointment. If it has been 12 months or more since your last chiropractic appointment the doctors will be required to perform a new examination at an additional charge to you.

 

  1. Virtual Consultations: Virtual consultation fees must be paid in full prior to your appointment.

 

  1. Missed Appointments: Your account will be subject to a no-show charge of $50.00 for a missed appointment not cancelled with a 24 hour notice. If you are more than 15 minutes late to your scheduled appointment you may incur the $50 charge and will need to reschedule your appointment. These charges will be your responsibility and billed directly to you.  Please help us to serve you better by keeping your regularly scheduled appointments and arriving on time.

 

  1. Participation in Insurance Plans: We do NOT participate with ANY insurance plans, including Medicare. Therefore, we also do not submit any claims to insurance companies. If you have out of network benefits, it will be your responsibility to submit any claims to your insurance company. Ask the front desk for a Superbill if you would like to submit your claims personally.

 

  1. Medicare: We are NOT a Medicare provider. If you have Medicare and wish to be seen in our office you are forfeiting your right to submit for reimbursement. All treatment fees are your responsibility and must be paid in full at the time of service.

 

  1. Supplement Returns: We will only accept supplement returns within 30 days after the original purchase, as long as the bottle is un-opened and in good condition. Products that were shipped will not be returned unless damaged. Products not purchased through IHS will not be accepted.
